Job summary

Encore Communities is seeking a Certified Nursing Assistant to provide personal care for residents in their homes under the supervision of the RN. You will assist during admissions, transfers, and discharges and help patients follow their care plans with compassionate support.

Candidates must hold a CNA license, current CPR, and fingerprint clearance. Six months of CNA/LNA experience and LTC/SNF or Transitional Rehab exposure are preferred.

Qualifications

• CNA license is required before hiring.
• Must have current CPR and fingerprint clearance.
• Minimum of six (6) months CNA/LNA experience preferred.
• Recent Long-Term Care/SNF or Transitional Rehab experience.
• LTC/SNF/Rehab experience preferred.

Responsibilities

• Assist guests/patients during admissions, transfers, and discharges per care plan.
• Review care plans daily to determine changes in the patient’s daily care routine.
