Job summary

CareBuilders at Home – Sandy UT is seeking a compassionate Certified Nursing Assistant (CNA) to provide essential in-home personal care, support routines, and coordinate with care teams to preserve client independence and dignity. Ideal candidates will have a state-approved CNA program, unrestricted CNA certification, at least six months of healthcare experience, and current CPR.

Qualifications

• Completion of a state-approved CNA program and/or successful passing of a competency evaluation per state regulations.
• Meets state-specific certification requirements and possesses unrestricted certification as a CNA.
• At least six months of work experience as a CNA in a healthcare setting.
• Current CPR as required by client facility; other health and screening tests as required by specific facilities.

Responsibilities

• Complies with CareBuilders at Home policies/procedures.
• Complies with client facility nursing policies/procedures.
• Maintains confidentiality relative to patient care and HIPAA.
• Communicates information effectively to appropriate personnel.
• Documents patient care as assigned in accordance with facility policies and procedures.
• Follows the patient's assigned plan of care.
• Delivers personal care services to patients as assigned.
• Records patient input and output as assigned.
• Maintains competency by participating in continuing education programs and meets state-specific requirements.
• Provides patient care in a non-judgmental, non-discriminatory manner protecting autonomy, rights and dignity.
• Performs other duties as assigned.
