US Skilled Serve is seeking a Certified Nursing Assistant in Buena Park, CA. This full-time, on-site role requires CNA certification, subacute experience, and the ability to lift up to 50 lbs while assisting residents with daily care.

Responsibilities include documenting care, monitoring residents, assisting with transfers, meals, and hygiene, and maintaining a safe, respectful environment per facility policies.

Qualifikationen

• Must be a currently certified CNA according to state laws.
• Experience in subacute care preferred.
• Ability to lift up to 50 lbs and assist with transfers.

Aufgaben

• Record entries on flow sheets, notes, and charts when applicable.
• Report significant changes in resident’s condition to an LVN/RN as soon as practical.
• Do not disclose resident’s protected health information and promptly report suspected or known violations to your supervisor.
• Monitor a new resident’s room readiness and ensure bed, name tag, etc. are in place.
• Introduce yourself, roommates, and other staff to new residents.
• Mark and inventory resident possessions as instructed.
• Assist with transferring residents as necessary.
• Perform duties per state, federal, and facility policies and procedures.
• Follow schedules/assignments and cooperate with facility staff.
• Create warmth and a calm environment for residents.
• Report complaints and grievances to appropriate personnel.
• Notify supervisor if late or absent; report occupational incidents.
• Assist with daily oral care, bathing, dressing, grooming, and hygiene.
• Turn bedfast residents every two hours; assist with mobility and transfers.
• Observe and report skin, bowel movements, and changes in patient condition.
• Prepare residents for meals and assist with feeding when indicated.
• Record food/fluid intake and report changes in eating habits.
• Keep call button within reach and report hazardous conditions.
