Job summary

Hickory Grove Post Acute in Gallatin, Tennessee is seeking a Certified Nursing Assistant (CNA) to join our compassionate team. The CNA will provide direct patient care, assist with ADLs, support mealtime activities, and help maintain a safe, clean living environment for residents.

You will monitor residents’ conditions, report changes to leadership, and work closely with nursing staff to develop and execute personalized care plans, building strong relationships with residents and families.

Qualifications

• Must be certified as a Tennessee CNA.
• High school diploma or equivalent is required.

Responsibilities

• Provide direct patient care, including assistance with ADLs such as bathing, dressing and grooming.
• Monitor and report changes in residents' conditions to leadership staff.
• Assist with mealtime, including serving and feeding residents.
• Maintain a clean and safe environment for residents and living areas.
• Develop and maintain positive relationships with residents, families and colleagues.
• Collaborate with nursing staff to develop and implement care plans.
