Qualifications

Responsibilities

• Identify residents before administering treatments using wristband or photo ID.
• Report all changes in the resident's condition to the Nurse Supervisor/Charge Nurse promptly.
• Record all entries on flow sheets, notes, and charts in a descriptive way.
• Maintain confidentiality of resident care information and report any unauthorized disclosures.
• Assist residents with daily living activities and mobility; help with transfers and transportation as needed.
• Prepare residents for medical tests (lab work, x-ray, etc.).
• Assist with lifting, turning, and repositioning residents; provide comfort and safety.
• Follow policies on exposure to blood/body fluids and use of protective equipment.
• Keep resident rooms clean and organized; ensure supplies and equipment are ready for use.
• Participate in nursing reports and shift handovers; communicate changes to the team.
