• Current Certified Nursing Assistant (CNA) certification in good standing.
• High School Diploma or GED.
• Valid driver's license and reliable transportation.
• Ability to complete all required training and screenings.

Responsibilities

• Administer medications and prescribed treatments per physician orders and agency policies.
• Assist individuals with activities of daily living (bathing, grooming, dressing, toileting, oral hygiene, meal assistance).
• Support mobility through lifting, transferring, repositioning, and ambulation assistance.
• Implement approved behavioral support strategies and crisis interventions when needed.
• Accompany individuals to medical appointments and community activities.
• Support ISPs and individualized care plans.
• Monitor health, safety, and environmental concerns and report issues promptly.
• Maintain a clean, safe, welcoming environment for individuals served.
• Complete required documentation accurately and timely.
• Collaborate with RN, Program Director, families, and interdisciplinary team.
