Job summary

Mentor Community Services, part of the Sevita family, is hiring a Certified Nursing Assistant to provide compassionate, hands-on nursing care and daily living support to individuals we serve.

As a full-time team member you’ll monitor health conditions, assist with personal care, maintain progress notes, participate in trainings, and contribute to a respectful, inclusive environment across our nationwide network.

Qualifications

• High School diploma or GED
• CNA certification program completed; current CNA certification issued by state
• One year CNA experience preferred; ideally with medically complex patients
• Current in Basic Life Support (BLS), CPR, and First Aid certifications
• Ability to effectively communicate with staff, residents, and families
• Reliable, responsible attitude with a compassionate approach
• Commitment to quality in everything you do

Responsibilities

• Monitor health conditions of individuals served and ensure quality services
• Provide assistance and supervision in daily living, personal care, and community/social integration
• Help maintain safe and quality supports for individuals served
• Initiate emergency procedures as needed
• Use equipment correctly and safely
• Report hazardous conditions, accidents, or incidents to supervisor
• Maintain progress notes and documentation
• Assist with resident activities and off-site appointments
• Assist with household activities
• Prepare rooms for admissions and discharges
• Attend and participate in team meetings and training
• Maintain confidentiality and respect for all individuals
