Job summary

Landmark Assisted Living in Vernal, UT is seeking a Certified Nursing Assistant to provide direct resident care under supervision of the Resident Care Coordinator. Duties include assisting with activities of daily living, feeding, mobility, and documenting intake and weights.

Strong team collaboration and adherence to care plans are essential. The role requires a valid CNA, CPR/BLS, a high school diploma, and a food handler permit where applicable.

Qualifications

• Certified Nursing Assistant in good standing with the state (preferred, not required).
• High school diploma or equivalent.
• Current CPR/BLS certification
• Current Food Handler Permit (when applicable)
• At least one year of experience as a CNA in a Skilled Nursing Facility or Assisted Living Facility is preferred, not required)

Responsibilities

• Provides supportive services to nurse, med tech and other staff as needed and performs duties as assigned.
• Assists resident with activities of daily living per care plans and policies.
• Assists resident with lifting, turning, moving, positioning, and transporting into beds, chairs, baths, wheelchairs, etc.
• Coordinates dining room services at mealtimes, including set-up, delivery, feeding, and documenting intake.
• Delivers nutritional supplements and documents intake.
• Assists with weighing residents and reports changes to nurse.
• Monitors skin condition and reports issues to nurse.
• Assists with toileting and completes flow sheets daily.
• Keeps supervisor informed of factors affecting tasks.
• Reports changes in resident condition to nurse and supervisor.
