Job summary

Ascend Hospice & Palliative Care NJ is hiring a Certified Home Health Aide (CHHA) to support patients in their homes. The role requires providing essential ADL assistance, hygiene, grooming, and comfort care under the supervision of a Hospice Nurse.

Responsibilities include mobility support, meals, vital signs, medication reminders, and thorough reporting to the nursing team. A High School Diploma and a State CHHA license are required; willingness to travel and reliable transportation are

Qualifications

State Certified CHHA license required.
Experience in hospice or home health preferred
Must be willing to travel and have reliable transportation
High School Diploma or equivalent

Responsibilities

Provide or assist with ADL activities including personal hygiene, grooming, mouth, skin care and nails.
Assist with toileting, positioning, transfers and turning for bed-bound residents/patients.
Support prescribed exercises and ROM as taught by Hospice Nurse.
Assist with light housekeeping and patient laundry.
Feed and assist with meals.
Take temperature, pulse and respirations under Nurse supervision.
Report changes to Hospice Nurse and document appropriately.
Assist or remind patient to take medications per regulations.
