Sapphire Health Services in Clatskanie, Oregon is seeking a compassionate caregiver for our community-based facility. You will assist residents with daily activities, provide companionship, and help maintain their safety and well-being onsite.

This entry-level role requires a HS diploma and a genuine desire to help others. Join our team to collaborate with care professionals and families, document care, and support residents’ independence while following established safety protocols and daily

Qualifications

• High school diploma or equivalent required.
• Experience in caregiving, home care, or community-based care preferred.
• Ability to work independently and as part of a team.
• Must be able to pass a background check.
• Ability to perform physical tasks such as lifting, bending, and assisting clients with mobility.
• Good organizational and time-management skills.
• Able to stand, walk, and reach for extended periods.

Responsibilities

• Provide assistance with activities of daily living (ADLs).
• Monitor and report changes in residents health.
• Assist with serving resident meals.
• Provide companionship and build relationships with clients to reduce isolation.
• Maintain accurate records of care provided (logs, notes, incident reports).
• Maintain resident confidentiality.
• Encourage participation in activities promoting physical, mental, and emotional well-being.
• Support clients in maintaining independence in daily routines.
• Ensure a safe environment by addressing potential hazards.
• Respond to emergencies per established protocols.
• Collaborate with care team, healthcare professionals, and families for coordinated care.
• Communicate care plans and changes clearly to residents and families.
• Present for staff meetings and trainings.
• Other duties as assigned.
