Job summary

StoryPoint Group is seeking a Caregiver/Medication Manager to provide compassionate care for residents in our community. You will assist with daily activities, perform medication-related tasks, and help residents maintain dignity and independence in a supportive environment.

The role emphasizes safety, adherence to care plans, and collaboration with nursing staff to ensure residents’ wellbeing and comfort in everyday routines.

Qualifications

• High school diploma or GED preferred.
• Ability to obtain Medication Manager certification on-site if not already certified.
• Healthcare, geriatric, or elderly population experience preferred.
• Desire to work with geriatric population.
• Training per state requirements.
• Current CPR and First Aid certification.

Responsibilities

• Bathe and clean residents as needed.
• Follow medication administration protocols for each resident.
• Assist in grooming and personal care for residents.
• Make beds, change linens, and keep rooms tidy.
• Respond to emergency calls promptly.
• Assist with bedpan, urinal, and bathroom routines; monitor and report changes.
• Record vitals, heights and weights as requested and report abnormalities.
• Assist with mobility and transfers; ensure safety during activities and ADLs.
• Maintain residents' privacy and dignity; uphold Residents' Bill of Rights.
• Participate in activity programming and care planning.
• Assist with admissions, transfers and discharge prep.
