Job summary

TEEMA Group is seeking a Caregiver in Fremont, California to provide compassionate, hands-on personal care to patients in home-based and center-based settings. Under supervision, you will support dignity, safety, comfort and independence according to individualized care plans.

You will perform grooming, bathing, meals, light housekeeping, and document changes for the care team, while maintaining a respectful, patient-centered approach and safety practices.

Qualifications

• Active CNA or HHA certification and BLS/CPR.
• 1 year of experience with elderly or frail populations.
• Ability to work independently and in a team.
• Strong interpersonal and communication skills.
• Basic English communication; documentation in English.

Responsibilities

• Provide personal care and escort assistance per care plan (grooming, dressing, bathing, toileting, meal prep).
• Observe and report changes in patient condition to care team.
• Demonstrate safe care practices and proper body mechanics.
• Interact respectfully with patients, families, and care team; encourage engagement.
• Complete routine care tasks accurately and consistently.
• Perform other related duties as assigned.
