Qualifications

• Must be dependable, punctual, and professional.
• Experience with dementia, hospice, mobility assistance is a plus.
• Able to follow individualized care plans and company policies.

Responsibilities

• Assist clients with ADLs including bathing, dressing and grooming.
• Provide companionship and emotional support.
• Assist with mobility, transfers and fall prevention.
• Prepare meals and assist with feeding when needed.
• Perform light housekeeping, laundry and household tasks.
• Provide medication reminders as permitted by policy.
• Assist with errands, shopping and transportation when authorized.
• Observe and report changes in clients’ conditions.
