Diablo Valley Post Acute in Concord, CA is seeking an organized Admissions Coordinator to manage the end-to-end admission process. You will liaise with hospitals, referral partners, and families to ensure a smooth transition into our post-acute facility, while supporting census growth and delivering exceptional service.

The role requires strong organizational and communication skills, attention to detail, and the ability to multitask in a fast-paced healthcare environment.

Qualifications

• Previous admissions, healthcare, customer service, or administrative experience preferred.
• Experience in a skilled nursing facility, rehabilitation center, hospital, or post-acute setting is highly desirable.
• Proficiency in Microsoft Office and EMR systems is a plus.

Responsibilities

• Coordinate all aspects of the admissions process from referral through resident admission.
• Communicate with hospitals, case managers, discharge planners, physicians, and referral partners.
• Welcome residents and families, providing exceptional customer service throughout the admission process.
• Verify insurance coverage and collect required admission documentation.
• Collaborate with nursing, therapy, social services, and administrative teams to facilitate timely admissions.
• Maintain accurate admission records and census reports.
• Respond promptly to inquiries regarding admissions and facility services.
• Assist with facility tours for prospective residents and families.
• Build and maintain positive relationships with referral sources and healthcare partners.
• Ensure compliance with facility policies and applicable federal and state regulations.
