Qualifications

• High School Diploma or GED required.
• Degree in human services preferred (Associate/Bachelor's/Master's).
• Qualifying experience in addiction, mental health, or related field.

Responsibilities

• Deliver recovery-focused support and daily living skills.
• Assist clients in accessing housing, medical care, education and supports.
• Collaborate with clinicians, peer specialists, and healthcare providers.
• Complete documentation and required reporting; conduct outreach when needed.
• Provide client transportation as clinically indicated; travel is essential.
