Job summary

S:US is seeking a Wellness Coach in New York to assist agency consumers with accessing community services and developing referral networks. The role emphasizes case management, crisis intervention, and supporting daily living skills within a collaborative social service team.

You will accompany clients to medical and social appointments, advocate for self-advocacy, and help create individualized support plans, while ensuring all documentation meets agency standards.

Qualifications

• Bachelor's degree preferred, not required, or 2 years experience with behavioral health population.
• Working knowledge of e-mail, Microsoft Word, Excel, PowerPoint and Windows OS.
• Two years' experience with individuals with mental illness, substance use issues and/or homelessness preferred.
• Experience in group work, tenant organization, case management, referral, and/or community outreach strongly preferred.
• Experience in supportive housing setting strongly preferred.
• Bilingual English/Spanish speaking preferred.

Responsibilities

• Provide case management to assigned caseload and monitor individual support plans.
• Assist with problem solving and crisis intervention.
• Accompany consumers to medical, psychiatric, and public benefit appointments.
• Advocate for consumer and support self-advocacy.
• Maintain daily living skills support and referrals as needed.
• Maintain relationships with supportive services to ensure quality care.
• Document all services in compliance with agency standards.
• Participate in supervision, team meetings, and training.
