Job summary

North Suffolk Community Services Inc. in Boston, MA is seeking a Community Support Specialist to engage and support individuals in housing, health, and community activities.

The role includes assessing needs, participating in treatment planning, and coordinating with housing and clinical teams to promote self-directed living. Responsibilities include facilitating social activities, assisting with medication administration per policy, and connecting clients with community resources.

Qualifications

• MAP and CPR/First Aid certification within five months of hire (training provided).
• Massachusetts Driver's License required.
• Bachelor's degree preferred; high school diploma/GED required.

Responsibilities

• Engage people and partner with them to improve health and wellness, achieve symptom stabilization and self-management.
• Gather information about community agencies to assist in service provision.
• Provide side-by-side support and coaching to help people socialize.
• Organize and lead individual and group social and recreational activities.
• Carry out treatment plan interventions and actions with direct support and rehab services.
• Collaborate with Housing Coordinators to help find safe, affordable housing.
• Participate in medication administration per MAP and DMH policies and protocol.
