Job summary
Boston Medical Center’s Living Well at Home Program is hiring a Senior Housing & Community Support Specialist to help high-risk patients access stable housing through intensive in-home and community-based services.
The role collaborates with housing providers, care teams, and social services to develop individualized housing plans, mitigate barriers, and support tenancy from intake to stabilization. It also leads a special project and mentors colleagues.
Qualifications
- Bachelor’s degree in a behavioral health or related field, or equivalent experience.
- Minimum 2 years in healthcare, public health, or social services.
- Experience with housing, homelessness, or community services preferred.
Responsibilities
- Engage high-risk patients in in-home and community outreach to support housing goals.
- Coordinate care with housing providers, care teams, and social services to implement housing plans.
- Maintain up-to-date documentation and participate in team case reviews.
- Lead or support a department project and mentor fellow specialists.
- Provide coaching and onboarding support to new Housing & Community Support Specialists.
