The Cahuilla Band of Indians seeks a Community Wellness Coordinator to deliver community-based intervention, case management, and resource navigation for Tribal members facing substance use concerns. The role supports engagement in treatment, coordinates transportation, and helps people access care and services.

Qualifications

  • Knowledge of substance use, opioid misuse, treatment, harm reduction, recovery, and related resources.
  • Knowledge of case management, service navigation, crisis intervention, de-escalation, and community response practices.
  • Knowledge of barriers to treatment and supportive services.
  • Knowledge of confidentiality and handling sensitive information.

Responsibilities

  • Provide community-based intervention, outreach, and case management to Tribal members.
  • Engage members and assist with identifying appropriate treatment and services.
  • Connect members to treatment, behavioral health, medical, recovery, and community resources.
  • Assist with treatment referrals, appointments, intake processes, and other connections.
  • Provide transportation to treatment and appointments when needed.
  • Maintain regular contact and follow-up with members in care.
