Job summary

Rincon Family Services in Chicago seeks a CC-JAC Case Manager to provide intake, needs assessments, service planning, and ongoing advocacy for individuals and families affected by community violence. You will collaborate with staff, connect participants to resources, document progress, and support program reporting and compliance.

The role requires trauma-informed care, bilingual capabilities, and strong communication within a diverse community setting.

Qualifications

• Experience providing case management or related services to individuals and families in violence prevention or social services.
• Knowledge of trauma-informed care and effects of community violence.
• Ability to assess needs, develop service plans, make referrals, and document progress.
• Ability to work with diverse communities and maintain confidentiality.

Responsibilities

• Conduct intake and needs assessments and develop individualized service plans with participants.
• Provide ongoing case management, follow-up, advocacy, and support to participants and families.
• Connect participants to community resources and partner agencies.
• Maintain accurate case notes and required program data.
• Participate in CC-JAC meetings, trainings, and program activities.
