Job summary

The Placement Coordinator at Brenham State Supported Living Center works under supervision to facilitate community transition planning and discharge activities for individuals moving to community settings. Qualifications

• Bachelor’s degree in human services is required.
• One year of experience working with individuals with intellectual disabilities.
• Ability to prepare reports and communicate effectively in writing and verbally.

Responsibilities

• Participates in and coordinates community transition planning (CLDP).
• Serves as liaison to staff, community providers and Local Authority staff.
• Ensures timely completion of required documentation within timelines.
• Assists with admissions, transfers, discharges and related duties as assigned.
• May train others and support policy/procedure development.
