Job summary

Bluebonnet Trails Community Services seeks a State Hospital Coordinator of Care to oversee continuity of care for clients transitioning in and out of the state hospital. The role supports diversion from jail and collaboration with multi-agency partners across eight counties.

The position emphasizes trauma-informed, person-centered care and uses BTCS EHRs to monitor referrals, missed appointments, and crisis episodes. Remote work requires approval; location is Williamson County.

Qualifications

• Bachelor's degree in social, behavioral, or human service field.
• Minimum of 6 months in case management, mental health crisis intervention or service coordination experience.
• Must be willing to work a flexible schedule.
• Must have skills using integrated computer database programs to access files and document treatment services.
• Must have valid Texas driver’s license and maintain driving status.

Responsibilities

• Ensure continuity of care for individuals transitioning to and from the state hospital.
• Identify candidates for BTCS diversion programs or outpatient treatment per protocols.
• Coordinate with jail personnel, courts, and DA offices to facilitate crisis intervention and jail diversion.
• Monitor health indicators and care plan progress using BTCS EHR and assessments.
• Link discharge teams with community resources to support step-down and graduation.
