We are looking for a dedicated and committed Community Support Worker who is striving to make a difference in the lives of the individuals we serve.

SUMMARY OF RESPONSIBILITIES
The primary role of a Community Support Worker is to help individuals acquire and maintain skills to improve their daily-living, domestic, adaptive, and socialization skills.

1. Provide 1:1 In-Home and Community Based support to individuals
2. Assist individuals with achieving a variety of goals in the home and local community
3. Transport individuals to medical appointments, personal shopping, and recreation activities
4. Cultivate safe and supportive relationships
5. Complete and Submit accurate weekly progress notes
6. Complete necessary training based on the Individuals' needs

REQUIRED SKILLS AND EXPERIENCE

1. Must be compassionate and willing to care for the well-being of others
2. Minimal of a High School Diploma or General Equivalency Diploma (GED)
3. Must have a valid driver's license and existing insurance
4. Must be willing to complete given background checks
5. Must be willing to drive Individuals in personal car throughout the community
6. Must have flexible availability
7. Excellent organizational, interpersonal, and communication skills
