Job Description - Mental Health Lead Support Worker (25820)

Position: Mental Health Lead Support Worker (Support Coordinator)

Type: Permanent, full-time (37.5 hours per week)

Salary: £13.21 per hour (£25,838 per annum) plus benefits including Health Cash Plan

Role Overview

Are you a night owl with a passion for making a difference? We are seeking a dedicated night worker to support our customers with complex mental health needs. Your role will involve creating and coordinating support plans, assisting with daily activities, and helping customers achieve their goals.

Key Responsibilities
  1. Develop support plans in collaboration with customers and coordinate a team of Support Workers.
  2. Assist customers with daily living activities such as meal preparation, shopping, budgeting, and maintaining tenancies.
  3. Conduct risk assessments, support planning, and regular reviews.
  4. Engage in meaningful conversations and build rapport with customers and colleagues.
What You Bring
  • Passion for supporting individuals to live their best lives with a collaborative approach.
  • Experience in creating person-centered support plans and supporting colleagues.
  • Experience in coordinating and assessing customer referrals.
  • Ability to work independently, stay calm under pressure, and demonstrate resilience.
