Job summary

Middle Flint Health & Wellness in Warner Robins, GA is seeking a full-time Community Support Team Lead to guide a multidisciplinary team in delivering high-quality behavioral health services to adults with severe and persistent mental illness in the community.

The role requires GA licensure (LPC/LCSW/LMFT), 3+ years of SPMI experience, and on-call availability for a 24/7 program; you will supervise staff, coordinate care, manage referrals and budgets, and champion evidence-based practices such

Qualifications

• GA board-licensed to practice independently (LPC/LCSW/LMFT)
• 3+ years with Severe and Persistent Mental Illness (SPMI)
• 6+ months in home/community settings
• Strong communication with community stakeholders
• Willingness for 24/7 on-call responsibilities
• Experience providing individual therapy and family interventions
• Knowledge of evidence-based practices (CBT, Solution-Focused Therapy)
• Ability to supervise and guide multidisciplinary team
• Ability to assess clinical needs and coordinate treatment
• Ability to manage caseloads, referrals, budgets, and contracts

Responsibilities

• Lead a multidisciplinary team including Case Manager, Peer Specialist, and RN
• Provide comprehensive, person-centered community rehabilitative services
• Deliver minimum 10 hours of direct client contact weekly plus individual therapy
• Coordinate treatment planning and supervise team members
• Monitor client health, clinical status, and treatment response
• Facilitate weekly team meetings and monthly staff meetings
• Oversee referrals, follow-up, and budget/contract obligations
