BrightPath LLC is seeking a Mobile Mental Health Specialist to support individuals with disabilities and mental health needs across the Twin Cities metro. The role emphasizes person-centered, hands-on care, skill-building, and independence within community settings.

The position requires travel, flexibility, and collaboration with care teams to ensure safe, dignified services. A 2-year Human Services degree and relevant experience are preferred, along with a valid driver’s license and background

Qualifikationen

• Minimum 2-year Human Services degree and 2 years of direct experience with IDD or related conditions.
• Valid driver’s license and access to reliable, insured vehicle; mileage may be reimbursed.
• Ability to pass criminal/registry background checks per Minnesota DHS guidelines.

Aufgaben

• Provide direct support to individuals with disabilities, mental health, or developmental needs in community settings.
• Develop personalized 24-hour Emergency Back-Up Plans for safety.
• Deliver mobile services across the Twin Cities metro to meet clients and train staff.
• Train and support new employees through shadow shifts to ensure quality service.
• Maintain documentation and report incidents to ensure regulatory compliance.
• Collaborate with waivered providers, mental health teams, social workers, and other professionals to coordinate care.
• Support crisis prevention and appropriate intervention during emergencies.
• Participate in 24-hour on-call rotation for emergency assistance.
