Job summary

Devereux Advanced Behavioral Health seeks a Community Support Professional to provide one-to-one services in homes, job sites, or community settings under an Individual Service Plan. You will document services for billing and regulatory purposes and record notes in Devereux’s EHR system, attending meetings as required by law and organization policy.

The position emphasizes training, transportation requirements, and a commitment to compliant service delivery.

Qualifications

• A High School diploma with at least six (6) months’ experience supporting adults with Autism OR one (1) year of experience supporting children/adolescents with Autism.
• BS/BA with at least six (6) months’ experience working with individuals with any developmental disability.
• Must be a minimum of 18 years of age.
• Must maintain a valid driver’s license and have access to a vehicle.

Responsibilities

• Develop, provide one-to-one interventions to an adult consumer at home, on the job site, or other community-based setting as defined by the Individual Service Plan.
• Provide and submit required documentation of service provision for legal/reimbursement purposes.
• Input and produce documentation in Devereux’s electronic health record (EHR) system in accordance with Devereux’s procedures.
• Attend review meetings with Devereux representatives as mandated by federal, state and local laws as well as Devereux.
• Participate in and submit documentation of training as directed by Devereux and/or mandated by federal, state and/or local laws.
• Provide current and valid driver’s license, vehicle registration and automobile insurance as required; transport clients under this agreement; ensure compliance and notify if non-compliant.
• Independent contractors will not be reimbursed the cost of mileage.
