Job summary

Community Solutions is seeking a Behavioral Health Therapist I to provide assessment, treatment and rehabilitation services to individuals with mental health disorders and potential co-occurring substance use. Under the Program Manager, you’ll deliver services across Santa Clara County in community-based settings.

Qualifications

• Master’s Degree in social work, marriage and family therapy, counseling, psychology or related field.
• Experience working with systems to meet requirements of the Agency and contracts.

Responsibilities

• Utilizes clinical best practices to conduct screening, intake and assessments to determine the client’s needs and develop treatment goals.
• Performs intensive counseling services with clients and families through individual, family and group therapy.
• Provides crisis intervention, counseling, advocacy, case management and referral services.
• Completes detailed program documents and maintains charts per agency and Medi-Cal regulations.
• Teaches life skills and connects clients to community services and rehabilitation counseling.
• Develops and maintains collaborative relationships with appropriate community organizations and system providers.
• Employee must have the ability to operate their personal or Agency motor vehicle to participate in client visits and transport.
• Responds to phone calls and emails within 24 hours on business days and participates in meetings for quality improvement.
• Maintains good customer service skills with internal and external customers.
• Completes training requirements and participates in on-call rotations.
