Job summary

TeamBuilders Behavioral Health, LLC is seeking a Community Support Specialist to join a multidisciplinary team providing Comprehensive Community Support Services (CCSS), Behavioral Respite Care (RSP), and Psychosocial Rehabilitation (PSR) in New Mexico. The CSS supports clients with Serious Emotional Disturbance, SMI, and COD, linking them to services and promoting recovery goals.

Responsibilities include collaboration with families and professionals, documentation, and service planning.

Qualifications

• Bachelor’s degree in human services or related field and 1 year of target population experience; or equivalent
• Certified Peer Support Worker (CPSW) credential is preferred
• Ability to obtain Criminal Records Clearance and maintain certifications (First Aid/CPR, CPI)
• Reliable transportation and valid driver's license required
• Strong documentation, communication, and teamwork skills

Responsibilities

• Provide CCSS, RCSS, and PSR services to individuals in home, community, or office settings
• Develop and maintain individualized service plans and progress documentation
• Coordinate with families, caregivers, and professionals; participate in supervision and team meetings
• Assist clients in accessing social services, housing, healthcare, and transportation
• Document activities promptly and ensure compliance with agency standards
