Job summary

Qualifications

• HS diploma or equivalent; excellent verbal and written communication skills and two years of driving experience required.
• Experience working with individuals with developmental disabilities, specifically in a community-based setting strongly desired.

Responsibilities

• Provide and/or assist with personal care and emotional support, helping individuals reach maximum independence as per their plan.
• Assist with meal preparation and daily living activities, maintaining a clean and healthful environment.
• Assist with medical appointments as needed.
• Monitor and report changes in individual's condition or living situation to the Program Director or Case Manager promptly.
• Maintain confidentiality of person-served information.
• Communicate with Case Manager and Program Director regarding schedules and needs.
• Engage persons-served in meaningful community or recreational activities.
• Attend required trainings and monthly staff meetings and supervision sessions.
• Complete and submit time sheets, logs, and mileage reports by deadlines.
• Other duties as assigned.
