Job summary

Mainstream Living, Inc. is seeking a Direct Support Professional to provide community-based services to individuals with mental and behavioral health needs in Des Moines, IA.

The role emphasizes safety, respectful communication, and partnerships to support recovery and personal goals. Responsibilities include implementing individual plans, coordinating with care teams, and assisting with daily living and medications as needed.

Qualifications

• High School Diploma or GED required.
• Valid driver’s license and insurable driving status in Iowa.
• Basic data entry skills.
• Post-hire training including medication administration.

Responsibilities

• Essential health care worker; may work one-on-one with members.
• Carry out steps in the individual plan and support recovery goals.
• Identify and facilitate opportunities reflecting members' skills and interests.
• Work with interdisciplinary teams and attend staffings; develop service plans.
• Meet member nutritional needs as per supervision plan and assist with meals.
• Complete member service documentation for billing and productivity.
• Provide instruction on medications and therapeutic intent.
• Link individuals to other support services.
• Support a nonjudgmental, inclusive culture.
