Responsibilities

• Provide compassionate, person-centered support in an outpatient urgent care setting.
• Engage clients during crisis visits to promote stabilization and treatment participation.
• Build trusting relationships using trauma-informed, recovery-oriented approaches.
• Connect clients with housing, transportation, financial assistance, healthcare, employment and other resources.
• Coordinate follow-up care and transport to appointments using your vehicle as needed.
• Document client interactions, referrals, progress, and services in the Carelogic EHR.
• Collaborate with therapists, nurses, prescribers, peer support specialists, and the team for coordinated care.
• Monitor client progress post-crisis and communicate changes to the treatment team.

Through a multi-disciplinary approach, the BHUCC team provides care to clients every day of the week, including evenings, weekends and holidays to meet the client's and community's needs.**What You’ll Do*** Provide compassionate, person-centered support to individuals experiencing behavioral health crises through in-person and telephone interactions in an outpatient urgent care setting.* Engage clients during and following crisis visits to promote stabilization, encourage treatment participation, and support successful transition to ongoing behavioral health services.* Build trusting relationships with individuals experiencing mental health and/or substance use challenges, using trauma-informed and recovery-oriented approaches to foster engagement.* Assist clients in identifying and reducing barriers to care by connecting them with community resources, including housing, transportation, financial assistance, healthcare, employment, and other supportive services.* Coordinate follow-up care and assist with transportation to appointments and community resources, using your personal vehicle as needed in accordance with client service plans.* Document client interactions, referrals, progress, and services accurately and timely within the Carelogic electronic health record (EHR).* Collaborate closely with therapists, nurses, prescribers, peer support specialists, and other members of the interdisciplinary team to ensure coordinated, high-quality crisis and follow-up care.* Monitor client progress following crisis intervention, provide ongoing encouragement and support, and communicate changes in client needs to the treatment team to promote safety and continuity of care.**Requirements of the Position*** Bachelor’s degree in a related field, or four (4) years of equivalent experience in lieu of a degree.* Valid driver’s license, current auto insurance, and a reliable, registered vehicle available for work use, including transporting clients.* Two (2)+ years of experience in a substance use, behavioral health, or related setting preferred.* Knowledge of community resources and health/social service systems in the Kansas City metro area preferred.* Strong ability to build relationships and engage clients in a recovery-oriented environment.* Excellent written, verbal, and interpersonal communication skills.* Proficiency in Microsoft Office (Word, Excel, Outlook) and general computer skills.**Schedule Requirements*** Full-time position, consisting of three (3) 12-hour shifts each week, plus one (1) 8-hour shift every other week.* Urgent Care operates seven (7) days per week; candidates must be available to work rotating weekends and holidays as needed.**Grow With Us**We’re committed to your professional development and will support your growth as a Community Support Specialist through training, collaboration, and meaningful work.**Ready to join a team that’s passionate about people?
