Job Summary:

As a Community Support Specialist, you will work with individuals across the lifespan to develop and implement personalized treatment plans that address mental health, behavioral, and social needs. Using evidence-based practices, you’ll help clients build resilience, connect with community resources, and achieve long-term wellness and independence. You will collaborate with clinical teams, schools, families, and community partners to ensure clients receive the support they need to succeed.

Key Responsibilities:

  • Provide community-based mental health services including assessment, treatment planning, and intervention

  • Collaborate with clients, families, and professionals to develop individualized care plans

  • Apply evidence-based interventions, including Motivational Interviewing, CBT, DBT, and trauma-informed care

  • Deliver behavioral interventions such as operant conditioning and interpersonal/group skills

  • Coordinate care with service providers, schools, healthcare professionals, and community agencies

  • Maintain timely and accurate documentation of services provided

  • Participate in training and staff development opportunities

  • Support quality improvement efforts through data collection and participation in program evaluation

  • Build and maintain effective relationships with clients and community stakeholders

  • Prepare for and lead PSR group sessions as needed

  • For ACT/ACT TAY/IMPART roles: understand ACT model principles and core processes

  • Perform other duties as assigned by supervisor

Education and/or Experience Qualifications:

  • Bachelor’s degree in Psychology, Social Work, or other human services field OR

  • Bachelor’s degree in an unrelated field with two years of related experience OR

  • Four-year combination of higher education and two years of related experience OR

  • Associate’s of Applied Science in Behavioral Health Support OR

  • Four years of qualifying experience*

*Qualifying experience includes delivery of services to individuals with mental illness, substance use, or developmental disabilities, including one-on-one or group services with a focus on recovery, skill-building, employment support, and community integration.

Required License/Certification (one of the following):

  • Valid driver’s license, proof of auto insurance, and acceptable driving record (if providing transport or traveling)

Additional Qualifications:

  • Knowledge of case management and rehabilitation practices

  • Understanding of mental health, substance use, and community resources

  • Strong written and verbal communication skills

  • Self-motivated with the ability to manage multiple tasks

  • Ability to engage effectively with clients and support systems

  • Skilled in collaboration, problem solving, and crisis response

  • Adequate written skills to complete timely documentation
