As a Community Support Specialist, you will work with individuals across the lifespan to develop and implement personalized treatment plans that address mental health, behavioral, and social needs. Using evidence-based practices, you’ll help clients build resilience, connect with community resources, and achieve long-term wellness and independence. You will collaborate with clinical teams, schools, families, and community partners to ensure clients receive the support they need to succeed.

Key Responsibilities:
Provide community-based mental health services including assessment, treatment planning, and intervention
Collaborate with clients, families, and professionals to develop individualized care plans
Apply evidence-based interventions, including Motivational Interviewing, CBT, DBT, and trauma-informed care
Deliver behavioral interventions such as operant conditioning and interpersonal/group skills
Coordinate care with service providers, schools, healthcare professionals, and community agencies
Maintain timely and accurate documentation of services provided
Participate in training and staff development opportunities
Support quality improvement efforts through data collection and participation in program evaluation
Build and maintain effective relationships with clients and community stakeholders
Prepare for and lead PSR group sessions as needed
For ACT/ACT TAY/IMPART roles: understand ACT model principles and core processes
Perform other duties as assigned by supervisor
Education and/or Experience Qualifications:
Bachelor’s degree in Psychology, Social Work, or other human services field OR
Bachelor’s degree in an unrelated field with two years of related experience OR
Four-year combination of higher education and two years of related experience OR
Associate’s of Applied Science in Behavioral Health Support OR
Four years of qualifying experience*
*Qualifying experience includes delivery of services to individuals with mental illness, substance use, or developmental disabilities, including one-on-one or group services with a focus on recovery, skill-building, employment support, and community integration.
Required License/Certification (one of the following):
Valid driver’s license, proof of auto insurance, and acceptable driving record (if providing transport or traveling)
Additional Qualifications:
Knowledge of case management and rehabilitation practices
Understanding of mental health, substance use, and community resources
Strong written and verbal communication skills
Self-motivated with the ability to manage multiple tasks
Ability to engage effectively with clients and support systems
Skilled in collaboration, problem solving, and crisis response
Adequate written skills to complete timely documentation
