Job summary

Qualifications

• High school diploma.
• Minimum of one year experience preferred.
• Valid driver’s license (minimum 3-year driving experience)
• Experience using electronic health record system preferred
• Minimum 21 years of age

Responsibilities

• Works towards the accomplishment of department/Agency goals.
• Maintains confidentiality and reports breaches in confidentiality.
• Completes required documentation accurately and within time frames.
• Observes professional boundaries with members, co-workers, and supervisors.
• Attends scheduled meetings as assigned.
• Completes all required trainings as assigned.
• Document progress toward ISP goals and objectives per guidelines.
• Review members' progress with Program Manager for new goals.
