Job summary

Tri-County Mental Health, DBA Beacon Mental Health is seeking a dedicated community support specialist to provide services to individuals with serious mental illness in community settings. The role involves crisis intervention, collaboration with the Health Care Home team, and development of strength-based, person-centered treatment plans to help clients achieve life goals.

Responsibilities include maintaining documentation in AVATAR, coordinating housing resources, and ensuring timely risk

Qualifications

• Associates or Bachelor's in Social Services; 4 years related work experience if no degree.
• Active driver's license and state-required auto-insurance.
• CPR/First Aid certification required.

Responsibilities

• Provide community-based services to assigned caseload with independent judgment.
• Deliver crisis intervention and recommend treatment including hospitalization when needed.
• Maintain contact with hospitalized clients and participate in discharge planning.
• Complete progress notes in AVATAR within 72 hours of service provided.
• Update Person-Centered treatment plans reflecting consumer goals.
• Coordinate housing resources and assist with basic needs and emergencies.
• Collaborate with Health Care Home team to support overall physical health.
• Engage reluctant clients and build a working alliance.
• Complete risk assessments and crisis plans as required.
• Attend team and all-staff meetings.
• Maintain a valid driver’s license and reliable transportation.
