Job summary

Qualifications

• Bachelor’s degree in social work, counseling, psychology, or a related field.
• Ability to relate effectively to severely mentally ill individuals.
• Must have, and maintain, a Communicare insurable driving record with transportation and valid license.

Responsibilities

• Provide case management services for clients who meet criteria for Community Support Services.
• Develop treatment planning and ongoing assessment of client needs.
• Coordinate case management with services provided by other agencies; provide supportive counseling.
• Assist CSS clients in applying for benefits and housing; help secure medications and develop medication management plans.
• Ensure access to mental health and community services; conduct home visits as needed.
• Maintain requested documentation; attend supervisor and program meetings; participate in grant activities.
