Job summary

Qualifications

• Bachelor's degree in Human Services or related field.
• No prior experience required.
• A valid Class D (IL) or Class E (MO) driver's license is required.

Responsibilities

• Provides community support services to complex clients following care plans and interventions.
• Completes timely documentation describing service need, intervention, dates, time, setting, and client response.
• Contacts individuals/referral sources after missed appointments to re-engage and support recovery.
• Provides mentorship and job shadowing to community support colleagues.
