Job summary

Essex Meadows in Essex, CT is seeking a part-time Community Social Worker to join our senior living community. You will coordinate resident care, liaise with families and physicians, and maintain confidential social service records to support residents across the continuum of care.

The role emphasizes wellbeing, access to internal and external resources, and coordination of short- and long-term stays within higher levels of care.

Qualifications

• Degree in Social Work, Gerontology, or a related field.
• Licensure as a Social Worker in Connecticut is preferred.
• 2–3 years of experience in long-term care, transitional care, memory care, or equivalent healthcare settings preferred.
• Proficiency in technology, including Microsoft Office Suite.
• Strong interpersonal and communication skills.

Responsibilities

• Support coordination of resident care and services across the community’s continuum of care.
• Serve as a liaison between residents, families, physicians, and community staff to ensure clear communication and coordination of care.
• Maintain accurate, current, and confidential social service documentation and resident records.
• Assist residents in accessing internal and external resources.
• Support Social Services in ensuring advanced directives are completed, updated, and properly maintained in resident records.
• Support resident wellbeing by assisting with wellness initiatives.
• Coordinate short-term and long-term stays within higher levels of care.
