Job summary

Valley HealthCare System is seeking Community Skills Trainers to support consumers in achieving wellness and daily living goals, including grooming, nutrition, cooking, and medication management. Duties include documenting care in EMR, following treatment plans, and participating in interdisciplinary team meetings.

Responsibilities also include ensuring safety, transporting clients as needed, training new staff, and completing mandated trainings.

Qualifications

  • High School Diploma or equivalent required.
  • Must be able to document services in EMR.
  • Ability to read, write, and speak English.
  • Valid Driver’s License required.
  • CPR, First Aid, and Mandt certifications required or obtainable.

Responsibilities

  • Facilitate and assist consumers in achieving wellness and daily living goals.
  • Complete EMR documentation for each daily episode of care.
  • Attend interdisciplinary team meetings and contribute to treatment plans.
  • Ensure safety and well-being of consumers on- and off-site.
  • Transport consumers to appointments and activities as needed.
  • Support training of new employees and participate in trainings.

Job description

ESSENTIAL DUTIES:As a role model, group leader, and motivator, Community Skills Trainers will facilitate and assist consumers in achieving goals of wellness and daily living skills (i.e. grooming, hygiene, nutrition, cooking, cleaning, taking medications appropriately, etc.).Documentation as required by Valley HealthCare System, Medicaid, OSHA, and other licensing agencies. Employee ensures that documentation (such as Direct Services Progress Notes, Client Transportation Progress Notes, etc.) are completed accurately and timely and will reflect services the client(s) has received services as appropriate.Perform all-predetermined services as per Individual Treatment Plan.Complete Electronic Medical Record (EMR) Documentation during each daily episode of care, notating, clinically, client’s progression/regression in each of the clients’ individualized treatment goals identified in Master Treatment PlanMay attend client Interdisciplinary Team meetings and participate in Treatment Plan developmentAssure that the safety, health and well-being of consumers are always maintained on or off site.Implement treatment/behavior/ plans in compliance with Valley HealthCare System’s and State guidelines.Completes and submits program specific documentation in a timely and accurate manner.Monitor, maintain and use appropriately, all equipment and supplies.Transportation of consumers to appointments, activities in the community, and to/from Program site, using a company vehicle or employee’s personal vehicle, on rare occasions.Support and help in the training of new employees.Attends in-service trainings, mandated meetings and completes Essential Learning trainings as assigned.Employee will engage consumers in achieving their daily goals and objectives through active treatment.OTHER DUTIES AS ASSIGNED.MINIMUM QUALIFICATIONS:Ability to perform Essential Duties as outlined below.High School Diploma or equivalent.Valid Driver’s LicenseAbility to obtain and maintain CPR, First Aid, and Mandt Certifications.Ability to comply with Consumer’s Rights.Ability to comply with Valley’s and Division departmental safety procedures.Ability to read, write and speak the English language.Basic Computer Skills (Typing, Microsoft Word)Ability to document services in an Electronic Medical Record (EMR)PREFERRED QUALIFICATIONS:BA/BS in Human Service Field or working towards this.Experience working with individuals with Intellectual/ Developmental Disabilities, Mental Illness and/or Substance Use DisordersAbility to work flexible hours on occasionExperience as part of a treatment teamPay: $14.25 per hourWork Location: In person
