Job summary

Valley Comprehensive Community Mental Health Center Inc in West Virginia seeks a dedicated direct care professional to provide skills training and supports for individuals with mental health/substance use diagnoses. Qualifications

  • CPR, First Aid, Mandt certifications or ability to obtain them.

Responsibilities

  • Provide skill development training and supports to persons served with mental health/substance use diagnoses.
  • Document services per Valley, Medicaid, OSHA and licensing requirements; complete progress notes and related forms.
  • Engage persons served in treatment plans, assist daily goals, and monitor health and hygiene.
  • Transport persons served to appointments when needed; complete shift reports and logs.
  • Attend trainings and comply with safety procedures and state/federal regulations.

Job description

NATURE OF WORK: To provide skill development training and supports to persons served with a mental health/substance abuse diagnosis. This involves person served assessment, program planning and implementation and documentation of services provided.ESSENTIAL DUTIES:As a role model and motivator, encourage good personal hygiene and health(i.e. grooming, bathing, dressing, nutrition, taking medications appropriately, etcDocumentation as required by Valley, Medicaid, OSHA, and other licensing agencies. Employee will ensure that documentation (such as Direct Services Progress Notes, Transportation Notes, 15-minute Checks, Behavior Tracking Forms, etc.) are completed accurately and timely and will reflect services the person served has received as appropriate.Perform all-predetermined services as per doctors orders.Assure that the safety, health and well being of persons served are always maintained on or off site.Implement treatment/behavior/ plans in compliance with Valleys and State guidelines.Employee will engage person served in achieving their daily goals and objectives through active treatment.Employee will ensure that personal care, change in person serveds health status and hygiene of all is maintained as person served needs dictate.Employee will complete all job duties as assigned within the specified timeframe or shift as scheduled. In doing such duties, all Valley Policies and Procedures as well as State and Federal regulations will be complied with in a promptly and timely manner. Examples of such standard duties include but are not limited to completing shower schedules, dressing and changing person served appropriately, preparing and following meals plans and schedules, notifying the supervisor of items needed by the persons served, completing shift to shift reports, following cleaning schedules, accompanying persons served to doctor appointments as schedule, completing van runs, reporting incidents to supervisors, completing van logs and following van procedures, completing work orders, etc.Complete and submits program specific documentation in a timely and accurate manner.Participate in person served Treatment planning.Monitor, maintain and use appropriately, all equipment and supplies.Transportation of persons served to appointments/activities in the community using a company vehicle.Support and help in the training of new employees.Attends in-service trainings, mandated meetings and completes Essential Learning trainings as assigned.OTHER DUTIES AS ASSIGNED.MINIMUM QUALIFICATIONS:Ability to perform Essential Duties as outlined below.High School Graduate or equivalent.Valid Drivers LicenseAbility to obtain and maintain CPR, First Aid, Mandt certifications and other required trainings.Ability to comply with Clients Rights.Ability to comply with Valleys and division departmental safety procedures.Ability to read, write and speak the English language.PREFERRED QUALIFICATIONS:Previous experience working with individuals with a mental illness and/or substance abuse.
