Job summary

St. Mary’s Center in Oakland is seeking a Community Services Case Manager to provide AAA-aligned case management for older adults and adults with disabilities enrolled in COS programs.

You will manage a caseload of about 30 participants, perform biopsychosocial assessments, develop care plans, coordinate services, and support Medi-Cal/CalFresh-related processes while working closely with community partners.

Qualifications

• Strong communication and interpersonal skills required.
• Experience working with older adults and vulnerable populations.
• Knowledge of Medi-Cal, SSI, IHSS, and CalFresh.
• Familiarity with trauma-informed care, motivational interviewing, and harm reduction.
• Understanding of elder abuse prevention and mandated reporting.
• Strong organizational and documentation skills.
• California Driver's License required.

Responsibilities

• Maintain an active caseload of seniors and adults with disabilities enrolled in AAA and COS programs.
• Conduct biopsychosocial assessments to identify strengths, risks, barriers, and service needs.
• Develop, implement, monitor, and update individualized case plans.
• Coordinate transportation, meals, medical and behavioral health services, medication support, and housing resources.
• Assist participants with Medi-Cal, SSI, CalFresh, IHSS, and related applications and renewals.
• Provide health-related care coordination and referrals.
• Conduct home visits and wellness checks; follow up as needed.
• Provide crisis intervention and de-escalation within scope of practice.
• Identify and report suspected elder abuse or neglect.
• Support transitions to higher levels of care and engagement in COS programming.
• Maintain HIPAA-compliant, timely documentation.
