Responsibilities

  • Conduct resident assessments to identify needs, strengths, and barriers to housing stability.
  • Develop individualized service plans and provide ongoing case management and follow-up.
  • Assist residents experiencing financial hardship, utility challenges, food insecurity, employment barriers, family issues, or other circumstances that may threaten housing stability.
  • Provide crisis intervention, supportive counseling, referrals, and resource navigation within the scope of the position.
  • Maintain regular communication with residents and monitor progress toward established goals.
  • Identify residents who may be at risk of eviction or housing loss and connect them with appropriate assistance.
  • Help residents access rental assistance, emergency assistance, utility assistance, food resources, and other stabilization programs.
  • Coordinate with property management, when appropriate and with appropriate confidentiality protections, to help address resident needs that may affect housing stability.
  • Connect residents with legal aid and other appropriate resources when they face housing-related legal issues.
  • Develop relationships with local nonprofits, government agencies, healthcare providers, workforce organizations, food banks, faith-based organizations, and other community partners.
  • Maintain an updated directory of community resources available to residents.
  • Make referrals and follow up to determine success in connecting residents with services.
  • Participate in community events, workshops, resource fairs, and resident engagement activities.
  • Maintain accurate, confidential case notes and resident service records.
  • Track referrals, outcomes, and resident participation in accordance with program requirements.
  • Assist with collecting data and preparing reports demonstrating resident services outcomes.
  • Maintain compliance with organizational policies, confidentiality requirements, and applicable professional standards.
  • Participate in staff meetings, trainings, supervision, and professional development activities.

Job description

The Case Manager provides on-site case management and supportive services to residents of an affordable housing community, helping individuals and families address barriers to housing stability, financial security, health, employment, and access to public benefits.

The Case Manager serves as a trusted resource for residents and works collaboratively with property management, community partners, government agencies, nonprofit organizations, and service providers to connect residents with appropriate resources and services. The position focuses on preventing eviction, strengthening resident stability, and improving quality of life while maintaining appropriate professional boundaries and confidentiality.

Key Responsibilities
Case Management & Resident Support
  • Conduct resident assessments to identify needs, strengths, and barriers to housing stability.
  • Develop individualized service plans and provide ongoing case management and follow-up.

Housing Stability & Eviction Prevention
  • Identify residents who may be at risk of eviction or housing loss and connect them with appropriate assistance.
  • Help residents access rental assistance, emergency assistance, utility assistance, food resources, and other stabilization programs.
  • Coordinate with property management, when appropriate and with appropriate confidentiality protections, to help address resident needs that may affect housing stability.
  • Connect residents with legal aid and other appropriate resources when they face housing-related legal issues.

Preferred:
  • Licensed Social Worker (LSW/LMSW) or eligibility for applicable state licensure.
  • Experience working in affordable housing, supportive housing, homelessness prevention, or resident services.
  • Experience working with seniors, veterans, families, children, or individuals with disabilities.
  • Bilingual or multilingual skills.

Physical & Work Environment

This position supports residents in our affordable housing communities and requires regular travel to our locations. Occasional travel to partner agencies, training sessions, community events, or other program locations may be required. Limited remote work may be permitted. The position is an essential member of the community impact team and plays a direct role in helping residents overcome barriers to stability, remain housed, and access opportunities that improve their overall economic and social well-being.

Compensation and Hours

Contractor basis. $41-$43/hour. Up to 30 hours per week.
