• Active New York State RN license required.

• Prior nursing experience required.

• Strong communication, organization and care coordination skills.
• Ability to work independently in field-based setting.

Responsibilities

• Coordinate and oversee healthcare services for individuals in group homes.
• Perform nursing assessments and clinical evaluations.
• Monitor overall health status and ensure continuity of care.
• Collaborate with interdisciplinary teams to support positive health outcomes.
• Schedule and manage care across assigned residential locations.
• Maintain accurate documentation and health records.
