Job summary

Matter Health is seeking aRegistered Nurse Care Manager to join our community-based team in the Boston area. You will partner with Nurse Practitioners and an interdisciplinary care team to deliver coordinated, relationship-focused care to older adults living in affordable housing communities.

Qualifications

• Active, unrestricted RN license.
• Minimum of 3 years RN experience.
• Experience working independently in community/home-based or field settings.
• Experience in care management or chronic disease management preferred.
• Strong communication, organization, and relationship-building skills.
• Experience with electronic health records (EHRs).
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct comprehensive nursing assessments in patients’ homes and community settings.
• Coordinate care with Nurse Practitioners and Medical Directors for high‑risk patients.
• Collaborate with specialists, primary care providers, hospitals and health plans.
• Educate patients on chronic disease management and preventive care.
• Support patients during transitions of care after hospitalizations.
• Identify and close care gaps through outreach and coordination.
• Document patient interactions and care activities in clinical systems.
• Build long-term relationships with patients and their families.
