Job summary

Neighborhood Health Centers of the Lehigh Valley is seeking a full-time Registered Nurse (RN) to join our growing team. The RN will be an integral member of the health center care team, working with providers, nurses, medical assistants, patient navigators, behavioral health specialists, and outreach staff to deliver high-quality medical care that supports health and wellness.

• Licensed RN with current licensure.
• Bilingual in Spanish preferred.
• Credentialing, employment clearances and vaccinations are required.

Responsibilities

• Provide high-quality medical care focused on assessment, promotion, maintenance and restoration of health and wellness.
• Assist with day-to-day operations and ensure appropriate access to care.
• Participate in care plan implementation and management for assigned populations.
• Ensure continuity of care and assist with care coordination.
• Serve as a clinical resource for other health center staff.
