Job summary

Trillium Health Resources is seeking a Community Resource Specialist I to help people access community-based resources addressing social drivers of health in North Carolina. You will connect individuals to services, track referrals, and coordinate with partners to ensure timely navigation and outcomes.

The role requires a high school diploma or GED with one year in health or human services, valid driver’s license, and the ability to travel within the region.

Qualifications

• High school diploma or GED and one year experience in a Health or Human Services field, including BH/SU/IDD populations; or equivalent combination of education and experience.
• Must have a valid driver’s license.
• Must reside within Trillium’s South-Central Region (counties: Carteret, Craven, Duplin, Jones, Lenoir, Onslow, Pender, Sampson, Wayne).
• Must be able to travel within catchment as required.

Responsibilities

• Receive and respond to referrals related to unmet health related resource needs, including housing, food, transportation, and employment.
• Connect individuals to community-based organizations and public programs to address identified needs.
• Maintain working knowledge of available community resources and service eligibility requirements.
• Coordinate referral follow-up to confirm connection to services and identify unresolved needs.
• Document referral activities, interactions, and outcomes accurately and within established time frames.

What We’re Looking For

Trillium Health Resources has a career opening for a Community Resource Specialist I to join our team! The Community Resource Specialist I supports access to community-based resources that address social drivers of health. The role connects individuals to services, tracks referral outcomes, and supports timely resource navigation through coordination with community partners.

Qualifications

Required:

• High school diploma or GED and one (1) year experience in a Health or Human Services field, including experience working the BH/SU/IDD population, including experience with public speaking, training and/or teaching; OR
• Equivalent combination of education and experience.
• Must have a valid driver’s license.
• Must reside within Trillium’s South-Central Region, which includes the following counties: Carteret, Craven, Duplin, Jones, Lenoir, Onslow, Pender, Sampson, and Wayne.
• Must be able to travel within catchment as required.

Preferred:

• Lived experience, community development experience, and/or experience with community-based organizations and/or community partners centered around housing, food insecurity, transportation, education & employment, interpersonal violence/toxic stress and/or social integration and inclusion
