Job summary

Citizens Memorial Hospital (CMH) in Stockton/Greenfield seeks a Community Resource Specialist to help patients navigate the healthcare system and access community resources. You will address social determinants to health in collaboration with an interdisciplinary team, ensuring patient safety and timely follow-up.

The role requires a high school diploma with strong communication, time management, and attention to detail; CHW certification within 6 months is expected, with preferred current CHW

Qualifications

• Ability to manage time and handle multiple referrals with attention to detail.

Responsibilities

Citizens Memorial Hospital (CMH) is recruiting aCommunity Resource Specialist to work with our amazing team in Stockton and Greenfield —someone who leads with skill, compassion, and a drive to positivelyimpactevery patient and family we serve.

• Status: Full-time
• WorkShift:Days
• Hours:Monday-Friday, 8am-5pm

What Your Day Looks Like: In this role you will support patients navigate the healthcare system, removing barriers to care, and facilitating community resources to address social determinants to health for patient safety.

As aCommunity Resource Specialist, you will be:

• Providing direct support to patients and families by addressing socio-economic factors impacting health, both in-person and by phone.
• Receiving referrals for specialty social service support for clinic patients and providing timely follow-up.
• Performing social needs screening and advising of applicable resources and solutions available.
• Documenting patient-centered plans of care.
• Collaborating and coordinating care with an interdisciplinary care team and advocating for patient safety in complex and challenging situations.

Qualifications

• Education: High school diploma
• Skills: Written and oral communication skills, time management, detail-oriented
• Licensure/Certification: Willingness to complete CHW certification within 6mo of hire ; Preferred: Current CHW Certification
• Physical Requirements:Prolonged periods of sitting to perform computer tasks
