Job summary

Qualifications

• Bachelor's degree in Social Work, Family and Human Services, Psychology, or related field.
• Experience conducting client needs assessments preferred.
• Comprehensive knowledge of community resources and nonprofit service systems.

Responsibilities

• Conduct comprehensive needs assessments for clients and identify barriers and opportunities.
• Develop customized resource plans with appropriate referrals.
• Provide clients with resource plans in their preferred communication method on the same day they are created or updated.
• Offer ongoing case management to help clients navigate resources and progress toward goals.
