Qualifications

• Bachelor's degree preferred in social services or related field.
• Associate degree with relevant human services experience will be considered.
• Minimum 3 years direct experience in SUD treatment.
• Knowledge of case management protocols and evidence-based practices.
• Experience with diverse and underserved populations.
• Strong written and verbal communication skills.
• Ability to manage multiple priorities and maintain records.

Responsibilities

• Engage clients from referral through treatment and coordinate care.
• Assist with benefits eligibility and enrollment in insurance programs.
• Provide resource navigation and advocacy for clients and families.
• Maintain accurate records and required reports; HIPAA compliant.
• Participate in interdisciplinary team meetings and care planning.
