Qualifications

• Three+ years of direct experience in Substance Use Disorder (SUD) treatment.
• Knowledge of case management protocols and evidence-based practices.
• Experience working with diverse and underserved populations.
• Fluency in Spanish and English (spoken and written).

Responsibilities

• Engage clients from referral through successful treatment completion.
• Coordinate care and support services to address client needs and treatment goals.
• Assist clients with benefits eligibility determination and enrollment in public and private insurance programs.
• Conduct social needs screenings and identify barriers to care.
• Provide resource referrals and service navigation assistance.
• Support clients and families in accessing clinical, behavioral health, and community-based services.
• Develop and maintain strong relationships with community agencies and referral sources.
